We investigate to learn, to implement preventive measures and prevent recurrence of adverse events. Investigating an incident is like peeling an onion. We start with the direct causes which are the most visible. To get to the bottom of the events we must uncover "layer upon layer" which others causes lay behind. The technological development, and new forms of organizations, increase the complexity and it becomes more difficult to understand the connection between the incident and the underlying causes. This course presents method, organization and process for the implementation of investigations. The course can also be offered in combination with a classroom session.
